This plugin is a simple GUI frontend which exposes Electrum's built-in swapserver (normally command line only) to everyday users.
This enables you to earn a yield on the BTC in your wallet. Other Electrum users who have BTC on-chain and want lightning (or vice versa) pay you to swap for them.
You set the rates.

No idea. Current swap rates I see in my electrum wallet are around 0.5%
0.5% per swap and 'no idea' how many swaps per year. technically a yield. practically a rumor.

Electrum's swapserver has been there for ages; the plugin~lightning community has covered this
Yes but previously you had to use the CLI to interact with it. This plugin enables any user to run a swapserver from the GUI. All the swap mechanics are handled by Electrum's own internal/reviewed swap server, this plugin just turns the swap server on/off and passes some settings through it.
