You're right that with BOLT12, the sender normally needs to manually approve each payment.
However, custodial wallets could easily automate this on behalf of the user.
Since the wallet controls the keys, it could fetch the invoice from the reusable offer and automatically pay it on a schedule — fully automating recurring payments.

If you're assuming a custodial wallet that you control, you can just not use lightning at all.
Here’s how it would work:

The user signs up on Orange Pill App (OPA) using their own wallet (e.g., Wallet XYZ).

Every month, OPA sends a reusable BOLT12 offer to Wallet XYZ.

If the user has funds available and has previously whitelisted OPA as an approved payee, Wallet XYZ would automatically fetch the invoice and pay it — no manual action needed
