Both the Sandy Hook and Washington Navy Yard shootings had early false reports of second shooters. It's not that they don't ever exist -- see Colombine -- but they get overreported early on.

Oh, in modern English, X is pronounced "Twitter".

Obviously this is a bit out of date (since it refers to Twitter), but I've always found this a good set of guidelines when something huge happens.
