Some years ago Bitfinex exchange supposedly had a hack (aka inside job), like many others. It's always an inside job, no matter they try to fool you.

So they offer to their users instead of g;ving back BTC some new system token named tether, supposedly backed 1to1 with USD. Practically creating fake money from thin air.
